Breast Implant Replacement

For many, the breast is a symbol of femininity. If you are unhappy with your breasts because they are too small, misshapen, or sagging, you can have breast augmentation or a breast lift performed by a specialist in plastic and cosmetic surgery. Cosmetic surgery can help women achieve their dream of having beautiful, feminine breasts. However, if you choose a certain cup size today, you may no longer be happy with the size or shape of your breast implants after a while. This means that you will need to have your implants replaced.

However, the change does not always have to be for purely aesthetic reasons. Some breast implants must or can be replaced after a certain period of time. There are three reasons for implant replacement. The first is a medical indication, due to capsular fibrosis and/or an implant defect (shell defect). This happens in very rare cases, as modern implants are of high quality. The second reason is aesthetic in nature, whereby the patient no longer likes the shape and/or size of the implants. The third reason may be a recall by implant manufacturers (Allergan, PIP, McGhan, Natrell, etc.).

Shape of Breast Implants

Round implants create a beautiful décolleté, but depending on the profile (height of the implant), they do not always look natural, as they do not always resemble the anatomical shape of the breast. The disadvantage of anatomical shapes is that they can rotate until they are completely fused with the breast tissue, which can affect their appearance.

In addition to the shape, there is also a difference in texture. The surface can be smooth or slightly roughened. The latter has the advantage that it integrates better with the breast tissue and thus significantly minimizes the risk of so-called capsular fibrosis.

Implants with silicone or saline solution?

Silicone implants filled with poly(organo)siloxane are extremely robust and durable. Silicone gel is an extremely safe material that has been used in medical fields for various treatments for many years. Thanks to its special composition, the breast feels natural.

Implants filled with saline solution are filled with sterile water. Specialist Dr. Münzberg first inserts the shell and then fills it with the solution. In the event of a defect in the implant, there is no danger to you. Unfortunately, the implants lose volume over the years as they lose water. In addition, unlike silicone implants, they do not feel as natural, even when placed under the muscle.

Position of the implants

Regardless of the material, a breast implant can be placed in front of or under the chest muscle. Both positions have advantages and disadvantages. The decisive factor is the existing tissue. If there is sufficient tissue (subcutaneous fatty tissue), the breast implant can be placed above the chest muscle. If you have less breast/adipose tissue, the implant will be placed under the muscle. This has the advantage of providing a stable system thanks to the support of the pectoral muscle, which can prevent the breast from sagging.

Necessary Breast Surgery

If capsular fibrosis with or without a shell defect is present, an implant replacement (en bloc capsulectomy with implant) must be performed and the implant with capsule completely removed. If the implant was placed over the muscle during the first breast implant replacement, it is recommended that it be placed under the muscle during the necessary reoperation. The reason for this is that the risk of capsular contracture recurring is significantly lower.

In the case of capsular fibrosis, the health insurance company covers 50% of the treatment costs (only removal of the capsule and implants) if the primary procedure was a cosmetic procedure. If the first procedure was completely covered by the health insurance company, all subsequent procedures are also paid for in full by the health insurance company.

Breast Surgery

The attending physician, Dr. Münzberg, removes the old, possibly defective implants and the capsule en bloc (i.e., the capsule that has formed is completely removed with the implant inside it, without opening the capsule) and replaces them with new implants. We only use breast implants from certified manufacturers of the highest quality: Polytech, a German manufacturer.

The operation takes approximately 2 to 3 hours. The surgical technique used by Dr. Münzberg will be discussed with you in detail during the preliminary consultation. He will also explain the possible risks to you.

Aftercare

Mr. Münzberg uses non-dissolving suture material, which means that the stitches need to be removed after breast surgery. The special suture technique enables optimal wound healing and minimal scarring.

Self-dissolving sutures often cause wound healing disorders and leave thicker scars, as these sutures have a high sugar content, which leads to increased scar tissue/granulation. After breast augmentation, you will need to wear a compression or support bra for several weeks. You should also refrain from sports and other physically strenuous activities, as well as sunbathing.
